次のとおり一般競争入札に付します。令和3年9月1日独立行政法人国立病院機構大阪刀根山医療センター院長 奥村明之進◎調達機関番号 597 ◎所在地番号 27１ 調達内容(1) 品目分類番号 26(2) 購入等件名及び数量 庁舎電力 予定使用電力量 4,600,000kWh(1年間予定使用量)(3) 調達案件の仕様等 入札説明書及び仕様書による。(4) 使用期間 令和4年4月1日から令和5年3月31日まで。(5) 需要場所 独立行政法人国立病院機構大阪刀根山医療センター(6) 入札方法落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額(当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする。)をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。(7) その他独立行政法人国立病院機構契約事務取扱細則(以下、契約細則という。)第22条の規定に基づき単価契約とする。２ 競争参加資格(1) 独立行政法人国立病院機構契約事務取扱細則(以下、契約細則という。)第５条の規定に該当しない者であること。なお、未成年者、被保佐人又は被補助人であっても、契約締結のために必要な同意を得ているものは、同条中、特別の理由がある場合に該当する。(2) 契約細則第６条の規定に該当しない者であること。(3) 厚生労働省競争参加資格(全省庁統一資格)「物品の販売」のＡ、Ｂ又はＣの等級に格付され、近畿地域の競争参加資格を有する者であること。(4) 電気事業法第２条の２の規定に基づき小売電気事業の登録を受けている者であること。(5) 環境配慮契約法に基づく二酸化炭素排出係数等による裾切り基準を満たす者であること。(6) 購入される電力を経理責任者が指定する日時、場所に十分供給することができることを証明した者であること。(7) 契約細則第４条の規定に基づき、経理責任者が定める資格を有する者であること。３ 入札書の提出場所等(1) 入札書の提出場所、契約条項を示す場所、入札説明書の交付場所及び問い合わせ先〒560－8552 大阪府豊中市刀根山5－1－1 独立行政法人国立病院機構大阪刀根山医療センター 契約係 浅野信之 電話06-6853-2001 内線2146(2) 入札説明書の交付方法 (1)の交付場所にて交付する。(3) 入札書の受領期限 令和3年10月22日 17時00分(4) 開札の日時及び場所 令和3年10月25日 15時00分 院内会議室４ その他(1) 契約手続において使用する言語及び通貨日本語及び日本国通貨。(2) 入札保証金及び契約保証金 免除(3) 入札者に要求される事項 この一般競争に参加を希望する者は、封印した入札書に入札説明書に定める資料を添付して入札書の受領期限までに提出しなければならない。入札者は、開札日の前日までの間において、経理責任者から当該書類に関し説明を求められた場合は、それに応じなければならない。(4) 入札の無効 本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる義務を履行しなかった者の提出した入札書は無効とする。(5) 契約書作成の要否 要(6) 落札者の決定方法 契約細則第21条の規定に基づいて作成された予定価格の制限の範囲内で最低の価格をもって有効な入札を行った入札者を落札者とする。(7) 詳細は入札説明書による。５ Summary(1) Official in charge of disbursement of the procuring entity:MeinosinOkumura,Director, National Hospital Organization Osaka Toneyama Medical Center(2) Classification of the products to be procured :26(3) Nature and quantity of the products to be required: Electricity of public officebuilding Contract: 4,600,000kWh(4) Fulfillment period : From April １,2022 through March 31,2023(5) Fulfillment place : Osaka Toneyama Medical Center(6) Qualification for participating in the tendering procedures : Supplierseligible for participating in the proposed tender are those who shall:① not come under Article 5 of the Regulation concerning Contract for NationalHospital Organization(NHO).
